About The Program
The Transitional Case Management (TCM) Program is an outpatient forensic mental health program that provides intensive case management to forensic clients. The TCM team works to facilitate the safe transition of clients from a secure forensic inpatient unit back into the community.
The team liaises with the Centre for Addiction and Mental Health (CAMH) Toronto Forensic Outpatient Services Department to provide psychiatry follow-up and case management, which focus on recovery, skill building and risk management.
Clients remain in the program until they are ready to transition out of the Ontario Review Board (ORB) system and are granted an Absolute Discharge. At this time, the team supports clients to transition to mental health supports in the community.
This community-based program is the final stage in the continuum of care within Forensic Services at CAMH.
Who's It For
Individuals who are living with mental illness, have been found Not Criminally Responsible (NCR) and are under the jurisdiction of the Ontario Review Board (ORB). Clients are living in the community, either independently or with appropriate supports (both formal and informal).
How IT Works
- Clients are under the jurisdiction of the Ontario Review Board.
- Program referrals are made directly from a forensic hospital (CAMH) or from CMHA Toronto’s other forensic programs: Transitional Rehabilitation Housing Programs (TRHP 1 and TRHP 2) and Forensic Supportive Housing Program (FSHP).
- Clients remain in the program until they are ready to transition out of the Ontario Review Board system and are granted an Absolute Discharge.
- The TCM team then supports clients to transition to generic mental health supports in the community if needed and desired by the client.

Who is the Transitional Case Management (TCM) Program for?
- Individuals who are living with serious mental illness
- Individuals transitioning from inpatient units and/or transitional housing programs to the community. These individuals are at a low to moderate risk of reoffence.
- Individuals who are under the jurisdiction of the Ontario Review Board as a result of being found Not Criminally Responsible, and who have been granted community living privileges in their disposition.
- Living in the community, either independently or with appropriate supports (both formal and informal).
