Recovery Housing Program

About The Program

The Recovery Homes Program supports seniors in transitioning from a shelter to independent living, and provides long-term services to improve their mental and physical wellbeing. Services are for seniors experiencing mental health and addictions challenges as well as chronic homelessness, and include senior-focused intensive case management, access to a rental subsidy and eviction prevention assistance.

Seniors experiencing complex mental health and addiction challenges, as well as chronic homelessness.

How IT Works

  1. Client is referred by the City of Toronto.
  2. Client is paired with a case manager.
  3. Client works with their case manager to create a plan for recovery, including finding housing and addressing primary and mental health care needs
