About The Program
The Recovery Homes Program supports seniors in transitioning from a shelter to independent living, and provides long-term services to improve their mental and physical wellbeing. Services are for seniors experiencing mental health and addictions challenges as well as chronic homelessness, and include senior-focused intensive case management, access to a rental subsidy and eviction prevention assistance.

How IT Works
- Client is referred by the City of Toronto.
- Client is paired with a case manager.
- Client works with their case manager to create a plan for recovery, including finding housing and addressing primary and mental health care needs
