About The Program

CMHA Toronto’s Nurse Practitioner (NP) Primary Health Clinics provide primary health care services to clients who are not connected to a health care provider. Recognizing that wellness encompasses both physical and mental health, individuals are fully supported by CMHA Toronto support staff and nurse practitioners, who collaborate closely to provide optimal care and treatment for physical health issues. The clinics aim to remove barriers to primary health care treatment by allowing for flexibility in scheduling, timely appointments and Ontario Telemedicine  Network (OTN) follow-up where appropriate.

How IT Works

  1. Clients inform their case manager that they’re interested in receiving primary health care services from a nurse practitioner. 
  2. The case manager will refer them to the clinic and schedule an appointment.
  3. Clients will continue to receive services from the NP Primary Health Care Clinic as long as they’d like, even after being discharged from other CMHA Toronto programs.

FREQUENTLY ASKED QUESTIONS

What is a nurse practitioner?

A nurse practitioner (NP) is a registered nurse who has completed additional training in order to provide health care to people of all ages. A nurse practitioner works in community health centres, hospitals, specialty clinics, rehab centres, nursing stations and long-term care homes.

Who is eligible for the Nurse Practitioner Primary Health Care Clinic?

Any client of CMHA Toronto, who is NOT connected to a health care provider because:

  • The primary care provider has moved to another community
  • The client has moved to another community
  • The client’s primary care provider is no longer available due to illness/death/retirement
  • The client’s primary care provider is no longer available due to change of practice type
  • Up until now, the client has not had, or felt they needed, a primary care provider

What services are offered by CMHA Toronto nurse practitioners?

In the primary health care clinic, our nurse practitioners can:

  • Perform a complete health history and assessment
  • Manage chronic conditions
  • Perform cancer screenings
  • Perform STI testing
  • Prescribe all medications
  • Order and interpret tests
  • Refer clients to specialists and other health care services
  • Provide immunizations
  • Complete forms (e.g., ODSP, special diet etc.)
  • Prevent health illnesses through screening tests
  • Provide health promotion, counselling and education

How long can you stay with the NP Primary Health Care Clinic?

You will continue to receive services from the NP Primary Health Care Clinic even after being discharged from other CMHA Toronto programs. Should you move, our nurse practitioner will assist you in finding a new health care provider.

How does one enroll in the NP Primary Health Care Clinic?

The first step is to inform your case manager that you are interested in receiving primary health care services from a nurse practitioner. Your case manager will refer you to the clinic and schedule an appointment for you.

Where are the clinics located?

The clinics operate out of CMHA Toronto’s East and West sites. 

What are the NP Primary Health Care Clinic hours?

Both clinics are open Monday – Friday, 9 am – 3 pm. 

Do you offer walk-in services?

At this time, we do not offer walk-in services.

Do you offer virtual care?

Telemedicine/videoconferencing may be available for some appointments.
