About The Program

Housing First Intensive Case Management (ICM) is a client-centered, recovery-oriented approach to ending homelessness. It is based on the principle that all people deserve housing, and that adequate housing is a precondition for recovery. Our top priority is helping individuals obtain housing, and providing case management support to maintain their housing and achieve their other desired goals.

The program supports individuals living with mental health and addiction challenges, and who are experiencing homelessness in Scarborough or North York. Apart from maintaining housing, individuals are supported through different areas of their life such as physical health, mental health, addiction support and making meaningful community connections. We have a multi-disciplinary team that includes a registered nurse, a housing worker, a behavioural therapist, a concurrent disorders specialist and more.  

We believe in working with individuals to help build meaningful community connections with appropriate supports that will assist with maintaining housing, fostering empowerment and independence, and achieving personal goals.

Individuals between the ages of 16 – 65 living with mental health and addiction challenges, and who are experiencing homelessness

How IT Works

  1. Clients are referred from The Access Point, an intake service that provides coordinated access for services in Toronto. Anyone can make a referral: the individual, their family, a physician or a community worker.
  2. Once accepted, the client is matched with a case manager.
  3. The case manager then works with the client to find housing
  4. The client continues to work with the case manager to ensure they keep their housing and create a plan for long-term recovery.

Who does the Housing First Intensive Case Management Program serve?

The program serves individuals 16–65 years of age who are:

  • homeless or precariously housed (e.g. unsafe, lack of privacy, lack of security) over an extended period of time (typically more than 3 months), or at risk of eviction. 
  • living with mental health issues (with or without a formal diagnosis). 
  • prepared to meet with program staff on a regular basis until new housing is secure and stable. 

What areas does the program serve?
