About The Program
CMHA Toronto’s Assertive Community Treatment (ACT) Teams provide treatment, rehabilitation and support services to individuals diagnosed with a serious mental illness who have had multiple psychiatric hospitalizations. The teams are multidisciplinary, with each having a social worker, nurses, case managers, a vocational/employment specialist, an occupational therapist, a psychiatrist, a peer support worker and an addictions specialist.
ACT Teams support individuals in their recovery process, meeting with them in their homes or in the community as frequently as the individual requires.

Individuals 16+ living with a serious mental illness who have had multiple hospitalizations
How IT Works
- The client applies through The Access Point, an intake service that provides coordinated access for Case Management services in Toronto.
- Once accepted, the client is matched with an ACT team member.
- The ACT team member then meets with the client and together, they identify goals and create a plan to advance recovery.

Who are ACT Team services for?
ACT Team services are for individuals who:
- Are 16 years of age and older
- Are living with primary diagnosis of a serious mental illness, with priority given to individuals living with schizophrenia (or other psychotic disorders) or bipolar disorder
- Are living with significant functional impairments, including the inability to perform daily living tasks, maintain consistent employment or maintain a safe living situation
- Have had recent psychiatric hospital admissions and who have high service needs
Where are ACT Team services offered?
ACT Team services are mobile and are offered in the community, in clients’ homes and in CMHA Toronto offices.
What areas do the ACT Teams serve?
CMHA Toronto has three ACT Teams, one in the West and two in the East, that serve individuals in Scarborough and North York.
Catchment boundaries:
- West ACT team: Steeles Avenue in the north, St. Clair West in the south, Humber River (Weston Road) in the west and Allen Road/Arlington in the east
- East ACT team: West of Victoria Park Avenue, East of Scarborough Pickering Townline, North of Steeles Avenue and South of Lake Ontario
- New Dimensions ACT Team: West of Victoria Park Ave, East of Scarborough Pickering Townline, North of Steeles Avenue and South of Lake Ontario
How do you apply for ACT Teams services?
All applications to CHMA Toronto’s Case Management services are through The Access Point, an intake service that provides coordinated access for all case management services in Toronto.
Referrals are typically made through professionals, but self-referrals are also accepted. An application form can be obtained by calling The Access Point at 1-888-640-1934 or by visiting their website at www.theaccesspoint.ca.
